Rates vary by insurance carrier and are billed as follows:

CPT code 90791-Initial Intake = $120.00
CPT code 90837-53 minute session = $100.00
CPT code 90834- 45 minute session = $ 85.00
CPT code 90837 – 30 minute session = $70.00

Clarity Counseling Services is currently in-network with some insurance carriers and is in the process of credentialing with other carriers. Current in-network carrier’s include:

• Anthem Blue Cross / Blue Shield – HMO ; PPO; Indemnity/Traditional/Standard
• United Healthcare
• Aetna

Therapy may be covered in full or part by your health insurance plan. Please check with your insurance carrier prior to the start of services. Below are some questions that will guide you in your conversation with your insurance carrier.

1. Do I have mental health coverage?
2. What is my deductible and or co-pay?
3. How many sessions do I get per year?
4. Do I need a referral from my primary care provider?

You will be responsible for your co-pay at the time of service and will be responsible for any service(s) not covered by insurance.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and haven’t notified us with at least 24 hours in advance, you may be required to pay the cancellation fee of $50.00.
