What happens when I call to schedule an appointment?

Your first contact with our office will be with our Office Manager, Susan. Susan will ask you a few questions and will answer any questions you have regarding services. If an appointment is scheduled, you will be sent an invite/link via email to create a portal user in our electronic health records system, Therapy Notes. You will be asked to fill out paperwork and upload pictures of insurance card(s). By filling out your paperwork prior to the scheduled intake session, it moves the intake process along much faster.

 If you do not have email and cannot complete the paperwork on-line, please be sure to arrive to your scheduled intake appointment 30 minutes ahead of your appointment time to fill out the intake paperwork. Also be sure to bring your insurance card and ID, so photocopies can be made at that time.

What happens during my initial intake?

An initial intake session may feel overwhelming, but it’s an opportunity for the therapist to gain knowledge and information regarding the clients concerns and history. It’s also a time for the client to determine if he or she feels comfortable and can connect with the therapist. During this initial session long term and short term goals of therapy are discussed as well as the wants and needs of the client.

What is your Confidentiality policy?

All health care providers are ethically and legally required to protect the confidentiality of their clients. Information will not be shared without a signed release of information from the client.

NOTE: There are conditions under which a client’s information is released and not protected.

  • Suspected child or elder abuse for which the therapist is mandated to report to the appropriate authorities immediately.
  • If the client is threatening serious bodily harm to another person. The therapist must notify the police and inform the intended victim.
  • If the client is a harm to him or herself, the therapist will make every effort to enlist the client’s cooperation to ensure their safety. If the client doesn’t cooperate, the therapist will take further measures without the client’s permission that are required by law to ensure the client’s safety.

Where are you located?

We are located near downtown South Bend, directly across from Everwise Credit Union (formerly TCU). Our address is 818 E. Jefferson Blvd. The main entrance is located in the front of the building.

Where should I park?

There’s parking available in the back parking lot, as well as in front of the building on Jefferson Blvd.
